I, the undersigned, hereby grant permission to Free Wigs for Kids and HairPlaceNYC (“Owners”) to post my and/or my child’s story, name, photo, video, or other item, hereinafter referred to as (“Materials”) to any Website, blog or social media channel operated by Owners. This permission is only granted for promotion of the philanthropic activities of the owners and not for private commercial gain.

I hereby release you, your representative, employees, managers, members, officers, parent companies, subsidiaries, and directors, from all claims and demands arising out of or in connection with the Owner’s use of said Materials, including, without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation and any other personal and/or property rights.

I acknowledge and agree that no sums whatsoever will be due to me as a result of the use and/or exploitation of the Materials or any rights therein.

I acknowledge that my child is under 18 years old and lacks the legal capacity to enter into binding agreements. Accordingly, I have read this Release and consent to my child’s inclusion in the Materials. I will not contest the rights granted in this Release.

By submitting this form, I certify that I have read and agree to the Video and Picture Waiver/Release Form. Submitting this form is the same as signing a paper form. I certify that I am either over 18 and agree to the Waiver or that if I am under 18 years old, my parent(s)/guardian(s) have given me permission to participate and have read and agree to this waiver/release on my behalf.

The parties agree that the undersigned may cancel this authorization at any time. The parties further agree that if the authorization is executed on behalf of a minor, the minor may cancel the authorization at anytime after the minor becomes an adult.
